To claim for re-imbursement of treatment costs which you paid cash for you should submit the following to Alliance.

Health for processing
Fully completed claim form
Original receipt of payment made
Referral letter (if service is by a specialist or for diagnostics)

A claim is only valid for 3 months from date of treatment after which is considered “expired” and cannot be presented for payment from the fund.

Pre-authorization is a process through which a member/service provider seeks approval from Alliance Health prior to undertaking treatment or incurring costs.

Pre-authorization is done to:

  • determine if treatment will be covered
  • determine is cover is with or without shortfalls
  • check availability of benefits

The length of the waiting period before you may begin claiming benefits is determined by the plan/scheme you have chosen. Specific waiting periods are detailed in your benefits table

Payments can be done at any of our branches in form of cash or card swipe as well as local and international transfers.

Shortfalls may be there on treatment is the service fee is more that the scheme/plan award for the treatment being received. We encourage you to seek preauthorization before treatment to get advice on shortfalls.

All subscriptions/ premiums are to be paid on or before the first of the invoiced month.
